PhilHealth reminds members to update records
The Philippine Health Insurance Corporation is reminding all members to update their Member Data Record, to avoid hassles in claiming for benefits.
The member must fill out the amendment form M2 and submit photocopies of appropriate supporting documents. In the declaration of dependents, the documents required are: for legal spouse – marriage contract/certificate with registry number; dependent children – birth certificate with registry number; and dependent parents – birth certificate of parent and birth certificate of member.
If a parent’s birth certificate is not available, any one of the following valid IDs is acceptable: senior citizen’s ID, voter’s ID, driver’s license, passport, or a notarized affidavit of two disinterested persons attesting to the date of birth of parent, attach to the Certificate of No Record found from National Statistics Office or Local Civil Registry.
The MDR is now considered a primary document in claim applications. PhilHealth is embarking on a campaign to have an accurate database, and reduce Return to Sender and doubtful claims.
Human resource and personnel managers are highly encouraged to facilitate the submission of amendment forms M2 and supporting documents by the employees to the PhilHealth office nearest them. (PR)
