Vinarao said an investigation conducted by the agency’s legal office showed there was gross neglect of duty on the part of  prison guards Joffrey Lim, Rolando Bugarin, Eric Pactorayan, Salvador Villadolid and Enrique Valenzuela that led to the escape of four prisoners last Oct. 9.
