Palpak na pamumuno dahilan ng pagsadsad ng USS Guardian

MANILA, Philippines - Palpak na pamumuno ang itinuturong dahilan sa pagsadsad ng USS Guardian sa Tubbataha Reef sa karagatan ng Palawan noong Enero 17.

Matapos ang limang buwan ay inilabas ng US Navy ang resulta ng pagsisiyasat nito sa pagsadsad ng barko noong Enero 17 na sumira sa 2,345.67 metro kuwadrado ng bahura.

Sa 160-pahinang ulat ng US Navy, kasalanan ng buong crew ng dating minesweeper ang insidente. Sinabi sa ulat na: "failure of leadership to provide adequate oversight and direction in planning and executing the Navigation Plan" ang nagdulot sa trahedya.

Binanggit din sa ulat na dapat ay “entirely preventable" ang insidente kung hindi lamang dahil sa palpak na mga tripulante ng barko.

Dadgad ng ulat na produkto ng "poor voyage planning, poor execution, and unfortunate circumstances," ang nangyaring insidente sa Tubbataha Reef na isang UNESCO world heritage site.

"This investigation uncovers no single point of failure; instead, there were numerous links in the error chain leading up to the grounding. Had any one of which been appropriately addressed, the grounding would have been prevented," sabi sa ulat.

"Finally, USS GUARDIAN leadership failed to exercise due diligence to ensure the watch teams were knowledgeable and proficient, and failed to recognize that key personnel transfers within the navigation team had degraded USS GUARDIAN's navigation capability to an unacceptable level. Ultimately, the lack of leadership led to increased navigational risk to the ship and her crew," paliwanag pa sa report.

Sa makapal na ulat, walang binanggit na parusa para sa mga tripulante ng dating USS Guardian na natanggal mula sa pagkakasadlak sa bahura noong Marso 30.

Sinibak na sa puwesto ang commanding officer na si Lt. Cmdr. Mark Rice, assistant navigator Lt. Daniel Tyler, at ang iba pang opisyal ng barko.
