Starting this month, all members of Philippine Health Insurance Corp. must attach a clear copy of their members’ data record in order for them or their dependents to avail of the benefits from the National Health Insurance of the Philippines.
It is in the Members’ Data Record that the names of the member’s dependents as well as his other personal circumstances are reflected.
PhilHealth assistant vice president Reynaldo Capangpangan informed that this development is embodied in the recently issued PhilHealth Circular No. 07 Series of 2007.
Capangpangan advised the members to secure a copy of their MDR from any PhilHealth regional or service offices nationwide.
In cases of discrepancy between the MDR and the member or dependent’s information provided in PhilHealth Claim Form 1, or even when the dependent is not reflected in the MDR, the member shall submit or attach to claim Form 1 properly accomplished Member Data Amendment Form (M2) together with clear copy of supporting documents as proof of dependency.
The M2 is available at any PhilHealth regional and service offices or hospitals.
Since what is required by the hospital during admission is only a clear photocopy of the MDR, all NHIP members are advised to keep the original copy of the document.
In order to avoid inconveniences, Capangpangan urged the PhilHealth members to strictly comply with the new directive.
